To cut down on some of the weeks of work it takes to turn your kitchen wizardry into a collection people can cook at home, we've crafted a cute form that you can quickly fill out.

Important Note: No promises, but production budget allowing, we'll accept photos of your dish or your family preparing it. If we manage to get them in, they'll be in black in white. Please send the files to sixthstreetparade@gmail.com (JPEG, high resolution), or submit originals to the office (for Melissa Armstrong). Either way, please include the name of the recipe and your contact info in case there's an issue.
